Do you need something SPECIAL?

Have you looked through our product selections and been unable to find what you are wanting? Walking Bird can create a tag exactly the way you are thinking about. If nothing we have matches the special form you are needing, try out our custom form template. Fill in the information, send it to us and we will contact you to discuss your request. Remember: if you can imagine it, we can make it. If the template doesn’t help you just shoot us an email and tell us exactly what you are looking for.

Your Custom Form

Walking Bird has many tags and forms ready to go. You can order them with your store information imprinted on the form or with no imprint at all.

You can also order, in larger quantities, a form with the same or similar characteristics but with the copy exactly as you want it. More and more stores require their own specific work descriptions and, in some cases, a SKU number associated with it that corresponds to the SKU in their POS System.

If you would like to receive a quote on your own Repair Form please fill in the blank areas that are indicated on the Step 1 tab.

Areas are as follows:

These “Fill-Ins” will populate and you will see an image of your redesigned form. With this and the information you provide on the Step 1 tab, we will have much of the information we need to create a quotation and contact you with any questions to help finalize your form design.

A. Number of copies: Three—two soft and one hard copy
B. Ink: One ink color - Blue
C. Numbering: Sequential—One large number and one small number.
D. Perforations and Holes: Two perforations and two hole punches as you see above
E. Back Print: Yes: Walking Bird’s standard back print copy
F. Quantity: Typically 5,000 to 20,000



A. Typically, there are three copies of the form with 2 of the copies being “soft” paper copies and the third copy being “hard” or card stock. Is the two soft copies and one hard copy format acceptable to you?

C. Typically, we number the forms sequentially with one small number (see at right) that can be seen on all three copies of the forms and we place one larger number on the Item ID Stub at the bottom of the form. Is this numbering format acceptable to you?

D. Typically, we have two horizontal perforations. One is at the top to separate the three form copies form the top stub and another is above the large number to separate the Item ID Stub form the main tag.
